Here is the honest picture. No muscle-loss study is open in Columbia today. However, this is the fastest moving corner of weight research, and that will change. So here is how to use this page.

  1. Read the two studies below. Neither is in South Carolina. Also, neither takes people already on a shot.
  2. Open More details. Inside, you will find the exact rules, so you can see for yourself where you stand.
  3. Put your name in anyway. Because that is how we know to call you the day a study opens near Columbia.
  4. Read the section below on what is known. Finally, it covers what you can do for your muscle while you wait.

Of course, checking never signs you up for anything. You are only seeing where you might fit.

Here is every muscle study we could verify. Neither is open to people taking a shot today. So read the rules carefully, and get on the list for the ones coming.

What exists right now, and where

Neither of these is in South Carolina. Also, neither one accepts people who are on a weight loss shot today. So we list them with the rules in plain sight.

Enobosarm, a Pill to Protect Muscle (PLATEAU)

Veru Inc. / Drug

Everyone here starts a weight loss shot, and half also take a pill meant to protect muscle. It is the nearest muscle study to Columbia, but the entry rules are strict.

More details

Why researchers are excited: it is the first large study asking whether a pill can keep your muscle while a weight loss shot takes the fat.

Who this study is looking for

  • Adults 65 and older
  • Also, BMI of 35 or higher
  • Besides that, weight under 350 pounds, because of the body scanner
  • Finally, able to climb a flight of stairs for the test

Probably not a fit if: you have taken a weight loss shot like Wegovy, Ozempic, Zepbound, or Mounjaro in the past 12 months. So if you are on one today, this study cannot take you.

  • Placebo group: Half the group gets the muscle pill, and half gets a placebo. However, everybody gets the real weight loss shot.
  • How long: About 68 weeks, so roughly 16 months.
  • Clinic visits: Not published on the public record. So ask the clinic when you call.
  • Phone calls: Also not published. But studies this size often add check in calls.
  • Most invasive part: A DXA body scan and blood draws. Also, you give yourself the weekly shot.
  • Setting: Outpatient.
  • Where it runs: Coastal Research Institute, Fayetteville NC 28304.
  • Drive from Columbia: Fayetteville is about 165 miles northeast, so roughly three hours.
  • Read this part twice: the 12 month rule is the one that surprises people. Basically, this study is built for people about to start a shot, not for people already on one.

Bimagrumab Plus Tirzepatide (Boston)

Massachusetts General Hospital / Drug

This study pairs Zepbound with a second drug built to hold on to muscle. It runs only in Boston, so we list it to show you what is coming, not as somewhere to drive.

Mid stage, dose finding Not near you

Est. about $100 per visit. Ask the site for the total number of visits.

Why researchers are excited: it measures muscle directly, by body scan, rather than guessing from the number on the scale.

Who this study is looking for

  • Adults 18 and older
  • Also, BMI of 30 or higher, or 27 and higher with a weight related condition
  • Besides that, at least one diet attempt that did not work
  • Finally, living close enough to Boston to attend in person

Probably not a fit if: you have diabetes, or you have used any weight loss medicine in the past 6 months. So again, current shot users are excluded.

  • Placebo group: Three groups run side by side. Notably, two of the three get an active drug.
  • How long: 52 weeks.
  • Clinic visits: Not published on the public record.
  • Phone calls: Also not published.
  • Most invasive part: A DXA body scan, an MRI, and blood draws.
  • Setting: Outpatient.
  • Where it runs: Massachusetts General Hospital, Boston MA 02114.
  • Why it is on this page: it is the clearest example of the science moving. Meanwhile, nothing like it has opened in the Carolinas yet.
  • What experts say:
  • “You always lose muscle mass when you lose weight.” Michael Blaha, MD, Johns Hopkins University (TCTMD)
  • “For elderly patients, because they've already lost muscle and bone throughout their lives and are at risk of issues like sarcopenia, frailty, and osteoporosis, we absolutely are concerned.” Michael Albert, MD, University of Oklahoma Health Sciences Center (TCTMD)

Why Muscle Matters Here in Columbia

The part nobody warns you about

Somebody loses a lot of weight fast, and everyone tells them how good they look. Then the stairs get harder. Next, they cannot get out of a low chair without a hand on the arm rest. Nobody uses the word sarcopenia for any of that, though. Instead, they say he got weak. They say she had a fall. Around Columbia that pattern is showing up more often, and the reason is not a mystery.

Why this is coming up now

West Columbia, Cayce, Irmo, Lexington, Northeast Columbia. Plenty of people across the Midlands started a weight loss shot in the last two years. The shots work, and that is not in question here. However, when weight comes off, some of it is muscle, and that is true of any weight loss. So the new question researchers are asking is simple. Can you keep the fat loss and hold on to the muscle?

So why is there nothing to join here yet?

  • The field is brand new. The first real muscle studies opened in the last year. So there are only a handful in the whole country.
  • They are small. These are proof of concept studies with a couple hundred people, not the huge trials the shots themselves ran.
  • Sponsors start where they already have sites. Right now that means Florida, Texas, and the Northeast. South Carolina is not on that list yet.
  • The rules exclude most people who care. Notably, the nearest study will not take anyone who has used a shot in the past year.
  • That will change. In fact, every major sponsor now has a muscle program. So the studies are coming, and we would rather you hear it from us first.

Research is a care option, not a last resort

There is a saying in medicine that research is a care option rather than a last resort. Generally, that is the honest way to think about a study. You are not giving up your own doctor. Instead, you are adding a second set of eyes. Notably, these muscle studies measure things a normal checkup never looks at, like how much of you is muscle.

What being on the list actually gets you

This is a directory, not a clinic. So being on the list is simple and it costs nothing.

What you actually get

  • A call from us when a muscle study opens near Columbia
  • The clinic name and number, so you call them yourself
  • A check against every other study on our list, not just this one
  • No cost, and no insurance needed
  • Roughly $100 a visit for your time, once you are in a study
  • You are never enrolled by checking

Add it up. Right now there is nothing here to join, and we will not pretend otherwise. Meanwhile, the science is moving quickly. So the only useful thing this page can do today is make sure you are not the last to hear.

What Is Actually Known About Muscle Loss on These Shots

The honest version, with both sides quoted.

Some Muscle Loss Is Normal, and That Part Is Not New

When you lose weight, some of it is fat and some of it is lean tissue. That is true of dieting, of weight loss surgery, and of the shots. As Johns Hopkins cardiologist Michael Blaha put it, you always lose muscle mass when you lose weight. So the question is not whether it happens. Instead, it is how much, and whether it matters for you.

How Much of the Loss Is Muscle

In one recent phase 2 study, the split was about two thirds fat and one third lean mass. The researcher presenting it called that consistent with previous weight loss studies, and reassuring. However, lean mass is not all muscle. It includes water and organ tissue. That is why the newer studies measure muscle directly with a body scan rather than guessing.

Where Experts Disagree

Some specialists are not worried. Samuel Klein of Washington University has said there does not seem to be a harmful effect of losing fat free mass when you lose weight this way. But others draw a line at age. Michael Albert has said that for elderly patients, who have already lost muscle and bone over a lifetime, doctors absolutely are concerned. In short, your age and your starting muscle matter more than the drug itself.

Why the Drug Companies Are Suddenly Interested

Every sponsor chasing weight loss is now also chasing muscle. The approach is to pair the shot with a second drug that blocks the signals telling muscle to shrink. Enobosarm and bimagrumab are the two furthest along. Notably, both are being tested as add ons rather than replacements, so you would keep the shot.

What You Can Do While You Wait

Nothing here is medical advice, and your own doctor knows your case. Still, the two things every specialist in these articles agrees on are protein and resistance training. Lifting something heavy twice a week does more for muscle than any supplement on a shelf. Also, ask your doctor for a body composition scan rather than just a weight. Basically, you cannot protect what you are not measuring.

Why This Page Exists Before the Studies Do

We would rather tell you the truth early than pad a page with studies you cannot join. Right now the muscle studies are small, and they sit in other states. However, this is the fastest moving corner of weight research. So if you put your name in, we will tell you the day one opens near you. Of course, checking never signs you up for anything.

What to Expect in a Muscle Study

New to studies? Here is how one works, step by step.

01

Prescreening

First comes a short phone call or online form. It takes about 10 to 15 minutes. The staff check if you might qualify. Then, if you do, they book your first visit.

02

Body Scan

Next comes a DXA scan. It is a quick, low dose body scan that separates fat from muscle and bone. Also, you lie still and it takes a few minutes.

03

Treatment Period

Then the study begins. You take the study drug or a placebo on the agreed schedule. Meanwhile, the clinic tracks your weight, your strength, and your scans.

04

Follow-Up

Finally, there is a check-in period after treatment ends. The clinic makes sure you are doing well. Also, they help you plan what comes next with your own doctor.

Muscle Loss on GLP-1 FAQs

Real answers about muscle loss on GLP-1 drugs near Columbia SC.

Is there a muscle loss study in Columbia right now?

No, and we would rather tell you plainly. Two studies are running in the country, and the nearest is in Fayetteville North Carolina, about three hours away. However, neither one accepts people who are already taking a weight loss shot. So if you are on one today, there is nothing for you to join yet. Meanwhile, putting your name in means we call you when that changes.

Do weight loss shots really cause muscle loss?

Some muscle comes off with any weight loss. That is true of dieting and of surgery, not just the shots. In fact, one Johns Hopkins cardiologist put it simply: you always lose muscle mass when you lose weight. So the real question is how much, and whether it matters at your age.

How much of the weight I lose is muscle?

In one recent study the split was about two thirds fat and one third lean mass. The researcher who presented it called that consistent with earlier weight loss studies. However, lean mass is not all muscle, because it also includes water and organ tissue. So the newer studies measure muscle directly with a body scan instead.

Should I be worried?

Experts genuinely disagree, and we will not pretend they do not. Some say there is no harmful effect from losing lean mass this way. But others draw a line at age, because older adults have already lost muscle over a lifetime. So talk to your own doctor about your age, your strength, and your starting point.

What can I do about it right now?

Two things come up in every expert discussion. First, eat enough protein. Next, do resistance training, which means lifting something heavy about twice a week. Also, ask your doctor for a body composition scan rather than just a number on the scale. Basically, you cannot protect what you are not measuring.

Do I have to stop my shot to join a study later?

For the two studies running today, yes, and that is the sticking point. The nearest one will not take anyone who has used a shot in the past 12 months. However, that rule exists because those studies are testing what happens from day one. So future studies built for people already on a shot are exactly what we are watching for.

What is sarcopenia?

It is the medical word for losing muscle mass and strength. It happens slowly with age, and it can speed up during fast weight loss. Notably, most people never hear the word from a doctor. Instead, they hear that they had a fall, or that they are getting weak.

Does it cost anything to be on the list?

No. There is no cost, and no insurance is needed. Tether is a nonprofit directory, so we do not run studies and we are not paid to refer you. Also, being on the list never enrolls you in anything. You would always call the clinic yourself.

How long until a study opens near Columbia?

We cannot promise a date, and we will not guess at one. However, every major sponsor now has a muscle program in development. Prisma Health and the University of South Carolina both run research here, so the infrastructure exists. In short, it is a question of when rather than if.

Where can I get help with muscle and weight near Columbia SC?

Prisma Health runs weight management programs across the Midlands. Also, a physical therapist can build you a resistance program that is safe for your joints. Your primary care doctor can refer you to either. Meanwhile, the University of South Carolina runs behavioral health research here, so it is worth asking what is open.
